Preliminary Requirements to get an Application Form

NURSERY 1 TO KINDER 2

Nursery 1 – Kinder 2

  • Pre-screening (N1-K1 only)
  • Latest Report Card (K2)
  • NSO Birth Certificate

GRADE SCHOOL AND HIGH SCHOOL

Grade School and High School

  • Latest Report Card
  • No failing grades in any subject, at least Conduct grade of B or its equivalent
  • NSO Birth Certificate

OTHER REQUIREMENTS: NURSERY TO HIGH SCHOOL

Other requirements for application – Nursery to High School

  • 2 pieces 1×1 identical pictures with collar and blue background
  • Baptismal Certificate (photocopy)

Procedure for Admissions

NURSERY 1 AND 2, KINDER 1 AND KINDER 2

NURSERY 1 & 2, KINDER

STEP 1. PRE-SCREENING
Bring the child and his/her NSO birth certificate to the Psychological Services Office for pre-screening.

STEP 2. APPLICATION FOR ENTRANCE EXAMINATION
For applicants who pass the pre-screening the Parents/Guardians may pay the testing fee at the Accounting Office and get the application form.

STEP 3. SUBMISSION OF FORMS AND REQUIREMENTS
Submit the accomplished Application form and other requirements to the Psychological Services Office and request for a schedule for the entrance examination.

STEP 4. SCREENING
Bring the child to the Psychological Services Office on the date and time the child is scheduled for the entrance examination.

STEP 5. APPLICATION STATUS
The Testing Services Office will contact the Parents/Guardians on the Admissions status of the application 15 working days after the entrance exams.

STEP 6. ACCEPTANCE
If accepted, pay a non-refundable/non-transferable reservation fee upon presentation of the Letter of Acceptance at the Accounting Office on or before the deadline indicated.

STEP 1. Present the following documents for tentative grade/year placement, as the case may be, to the Psychological Services Office.

a. If Filipino Citizen

  • copy of the NSO Birth Certificate together with the original
  • copy of Baptismal Certificate (Roman Catholic) of the child together with the original
  • original or certified True Copy of the Report Card (reflecting grades of current school year)

b. If Naturalized Filipino Citizen

  • copy of Identification Certificate
  • copy of Birth Certificate

2. Pay Testing Fee at the Accounting Office and get the application form.

3. Submit completed forms and requirements to the Psychological Services Offices at least four working days before the test date.

4. Applicant takes admissions test on scheduled date and time.

5. If applicable, an applicant will be given the schedule for interview by the Psychological Services Office.

6. The Testing Services Office will contact the Parents/Guardians on the Admissions status of the applicant 15 working days after the entrance exams.

7. If accepted, pay a non-refundable/non-transferable reservation fee upon presentation of the Letter of Acceptance at the Accounting Office on or before the deadline indicated.

Requirements for Admission (Foreign Applicants)

Requirements for Admission for Foreign Student-Applicants / Filipino Studying Abroad

  1. SUBMIT the following documents for evaluation to the SCHOOL REGISTRAR
    Incomplete requirements will not be processed.
  • If the applicant is NOT A FILIPINO citizen
  • If the applicant has DUAL CITIZENSHIP, submit a photocopy of PHILIPPINE PASSPORT and / or CERTIFICATE OF RECOGNITION as a Filipino citizen
    • Original PASSPORT
    • PHOTOCOPY of passport biodata page and latest admission with valid authorised stay
  • Parent’s Passport (Photocopy)
  • Alien Certificate of Registration (ACR) / I-CARD (Photocopy of front and back portion)
  • BIRTH CERTIFICATE OR FAMILY REGISTRY
  • COMPLETE & ORIGINAL SCHOOL RECORDS of all Grade Levels attended (from Pre-school to last grade level enrolled in); for NON-ENGLISH SPEAKING COUNTRIES, have the school records DULY AUTHENTICATED BY THE PHILIPPINE EMBASSY located in the student-applicant’s country of origin or legal residence, both in English and in his national alphabet
  • CERTIFICATE OF ENROLLMENT of the current school year or  CERTIFICATE OF COMPLETION of last completed school year
  • SPECIAL STUDY PERMIT (SSP) for a foreign student enrolled in the Philippines of the current school year or of last completed school year
  • 2 pieces of 2×2 most RECENT IDENTICAL PICTURES with white background
  • Place all the requirements in a LONG BROWN ENVELOPE
  • The school will inform the applicant of the status of his/her application after 5 WORKING DAYS from submission of requirements.
  • If the applicant qualifies for the entrance exam, s/he pays for the non-refundable application and testing fee at the Accounting Office. S/he will proceed to the Testing Services Office with the receipt and claim the application form.
  • The School Registrar will return the documents submitted of non-qualified applicants.

 

  • OPEN slots are on first come first served basis.
  • The student applicant may start applying for entrance examination after s/he has passed the interview with the Registrar.
  • END OF APPLICATION is on May 31, 2017
